Simple unsubscribe survey integrated into the unsubscribe process

Maybe just a drop down menu of 3 - 5 reasons why someone might be unsubscribing that is integrated right into the unsubscribe process. Doesn't send the user to a separate form or process and it's simple, simple, simple. We don't want to bug people who are already wanting to unsubscribe, but we do want to know why so we can address any recurring themes that arise.
