Whoever designed the mechanism that generates the PDF of the form data, chose to exclude the payment page.  The payment page lists all of the fees that we charge, and we can add fees based on rules.  Then, the user can either pay with a credit card, or choose "Bill me later".  By excluding the entire Payments page, we don't know what fees got charged (without looking back through the other answers), we don't know if the user wanted to be billed, and we don't know which credit card they used, if there's an issue.
